I participated in the audio conference on 10/7/2009.  It has been very helpful, I kept your slide presentation and have referred to it many times.  I have a quick question.  On:

Slide 21
•states Documentation for ‘incident to’ services must clearly link the services of the non-physician practitioner to the services of the supervising physician
Slide 22 
•states co-signature or legible identity and credential of both practitioner who provided the service and the supervising physician on documentation entries
•Notation of supervising physician’s involvement within the text of the associated medical recorded entry
•Documentation from other dates of service other than those requested, establishing the link between the two providers

Since attending your presentation I have been training my physicians/non-physician providers in this manner, however, I am looking for specific source document to site as we are drafting policies at this time.
